Now that you have completed the Setting up a Trigger Form, you are ready to start building .
You will need to begin by adding a question, this will always be the first step in creating your form.
Click ‘Add Question’. Entering the first question will trigger the rest of the form to appear. At this stage you will begin to build your form, adding the following elements:
- A title → Try to keep this short and concise so that it is clear for the recipient.

A description → This can be used to provide a little more context as to what the requirements of the form are and the purpose of completion.
Once you have entered the title, description and logo for your form. You can begin to populate the first page with questions.
Before we jump into question creation, let's mention Pages - our organisational structure within forms so that you can keep your questions grouped, explained and ordered. For example, in a new starter form, the first page can gather personal details, while the next page could have a different focus.
This ensures a logical and organised presentation for recipients.
Adding Questions
Adding questions to your form is super simple.
First, add a question and choose its question type. Then set the question title and give it a prompt if needed.
Question types
Use Add Question to add a question and then switch its format: click on the dropdown menu on the left, pictured below.
We will take a look at each of these options in a little more detail below.
Adding titles to questions
When you add a new question you will also need to write the actual question you would like to ask: What is your First Name?, Postcode, Choose your lunch option etc. Just click on the default title to edit it. You can also add a further prompt for your respondents by clicking Settings and adding a Description.
Question types
Checkboxes ☑️
Respondents click one or several checkboxes to select answers.
Use the Checkboxes type for questions that accept multiple answers.
Red options are what is shown in the form and the green options can be used to add additional options. Select All and Other (describe) options will always be available by default for you to add to a form as these are set up to select all options or to open a text box for additional information.
Dropdown ⬇️
Respondents select a single answer from a drop-down list.
Like Radio Button Group, the Dropdown type can be used for questions that have multiple options but accept only one answer. However, the dropdown menu can display more options while occupying less screen space.
File Upload 📤
Use the File Upload type to allow respondents to upload files.
Respondents drag and drop one or several files onto the allocated area or select files in the browser's Upload File dialog window.
Text Input 🔤
Respondents enter their textual, numeric, or date and time answers into a single-line editor.
Use the Text Input type for questions that require short answers.
Long Text 🔡
Respondents enter their lengthy answer into a resizable multi-line text area.
Use the Long Text type for open-ended questions that accept multi-line answers.
My Arbor Data
My Arbor Data is a question type that pulls existing data from the MIS for the user to be able to select. This field is compulsory when wanting to refer to the Students, Staff or Schools (if a MAT) within your MIS.
To use this the Question type the Text Input must be selected as My Arbor Data.
Once you have selected this you will see My Arbor Data appear in the question.
From here, select Settings on the question to open a slide over on the right-hand side. You will need to select the Arbor data that relates to the question.
You will see the option to select Staff, Student or School.
Once the form is deployed and the workflow is triggered the question will be visible to the user and will have a Select... drop-down option.
Having your question configured to My Arbor Data means they will be in the correct format to add a Arbor Data Step to automate communicate to certain areas within your MIS.
Arbor Option Sets
Once you have entered the question where you want to collect this data, select the 'text input' to open the drop-down of question type options. From here select Arbor Option Sets at the bottom of the list.
Once you have selected this you will see that 'Text Input' has updated to 'Arbor Option Sets'.
Next, select the 'Settings' button on the question. This will open a slide over on the right-hand side where you will now detail which option set you would like to be displayed when users are completing the form.
Once this has been selected you will see your option set selection displayed. Close the slide-over using the icon on the left hand side to save your format.
